PLEASE REMEMBER:
Upon completion of this form, you will have 5 consecutive days to complete the certification or you will need to register again, at your own expense. Your learners time starts the moment you submit their information using this form.
For example, if you register on a Monday at 10AM, the learner has until Friday at 10AM to complete the certification. If you register on a Friday at 10AM, the learner has until Tuesday at 10AM to complete.
